Both Dish and Direct offer local channels based on whether they can make money on uplinking them to the satellites or not. If no money is to be made, they'll offer another group. They also generally use a fiber optic path to get the signals back to the uplink site in Colorado (Direct) or Wyoming (Dish). There is a cost associated with that, too.
Sometimes they take signals direct off the air to feed the fiber while at other times, they connect directly at a studio or transmitter site.
Who has the best service and best systems? Kind of like Ford and Chevy, and everyone has a favorite. I know the Direct signal will more more robust in rain fades due to less path loss in this area, but who cares other than TIVO and a few techies?
