They have a website.....Google Jake's on Broad in Alex City (menu is linked on their site). Only been there twice, but not bad. I think I had a beer with dinner but can't remember.

What is the norm for dress, slacks/sport shirt?  I can't imagine a tie would be required, but at the same time wouldn't think jeans and a tank top would be OK.

I may have had on a collar shirt when we've went (that a reasonable bet), but I'm sure it was either jeans ,swim trunks, or shorts and flip flops on the lower end because thats all I ever wear around the lake.....casual dress is fine.

For lunch its pretty casual although they have club meetings there. This is a downtown restaurant that's casual but I would wear nice slacks or shorts and a knit shirt. Dinner is probably the same. It ain't Niffers.
